Dermatologists take a thorough and methodical approach when examining moles to ensure early detection of any potential skin cancers, including melanoma. A full skin assessment typically begins with a detailed history such as family risk, previous sun exposure, and any changes the patient has noticed followed by a head-to-toe visual examination.
During the examination, dermatologists look for moles that appear new, unusual, or different from a patient’s other marks (“the ugly duckling sign”). To enhance accuracy, many clinicians use dermoscope, a specialised magnifying device that allows them to see patterns and colours beneath the surface of the skin. This deeper view helps distinguish harmless moles from those that may require closer monitoring or biopsy.
The ABCDE Rule
A central component of mole evaluation is the ABCDE rule, a simple but powerful guideline used to identify features that may indicate early melanoma:
- A – Asymmetry: One half of the mole is unlike the other, suggesting irregular growth.
- B – Border: Edges appear notched, scalloped, or poorly defined.
- C – Colour: Variation in colour such as different shades of brown, black, red, or white may signal abnormal pigment changes.
- D – Diameter: Moles larger than 6mm (about the size of a pencil rubber) are more concerning, though melanomas can be smaller.
- E – Evolving: Any change in size, shape, colour, elevation, or sensation (such as itching, bleeding, or crusting) warrants professional review.
When moles display any of the ABCDE features, dermatologists may take digital images for comparison at future visits or recommend a biopsy to confirm the diagnosis. Early evaluation is crucial, as detecting melanoma in its earliest stages significantly improves outcomes.
Regular self-examination and routine skin checks with a dermatologist are the most effective strategies for maintaining skin health and ensuring that any concerning moles are identified and managed promptly.
